  • 1- A valid passport showing at least six months' validity from the date of entry in Brazil, containing one blank page;
  • 2- two Visa Application Forms, filled out, dated and signed by the applicant. Parents must sign for minors under 18 years old;

  • 5- a copy of a round-trip ticket or a booked itinerary showing travel to and from Brazil, confirming purchase of the ticket and passenger’s name, itinerary, flight number and arrival/departure dates;
  • 6- a copy of the US Alien Resident Card for foreign residents and/or a valid visa or document to return from Brazil or to go to another country;
  • 7- police clearance issued within the last 3 months, certifying absence of criminal record. Police clearance must be legalized by the Consulate. A fee of US$20.00 will be charged;
  • 8- letter of acceptance, legalized by a notary public in Brazil, from school, college, or university in Brazil stating length of studies;
  • 9- original Birth Certificate. The Consulate will then make a true copy. A fee of US$5 will be charged;
  • 10- the school, college or university must be registered at the Ministry of Education in Brazil. Proof of register must be notarized in Brazil;
